Obituary

Phyllis Slavik, nee Rakowsky, 83. Beloved wife for 63 years to Dr. Charles Slavik; devoted mother of Caryn (Michael) Loiben, Dr. Michael (Robin) Slavik, and the late Robyn Slavik; proud grandmother of Jared and Marlee Loiben, Brooke, Alexa, and Jack Slavik; caring sister of the late Marvin (Marilyn) Rakowsky and the late Sharon (the late Barry) Pass; loving aunt of Pam (Ron) Paglia, Shari (Solomon) Altit, Stacy Apter, Lori (Kyle) Kirl, Jodi Pass, and Marc Pass. Phyllis' life was her family and she will be deeply missed. In lieu of flowers, memorial contributions may be made to the Lung Cancer Research Foundation, www.lungcancerresearchfoundation.org. Chapel service Friday, August 30, 10 AM at Shalom Memorial Funeral Home, 1700 W. Rand Rd, Arlington Heights. Interment to follow at Shalom Memorial Park.
