Lucille Tolan nee Forman, age 88. Beloved wife of the late Philip Jay Tolan. Cherished daughter of the late Anna and Harry Forman. Loving mother of Michele (Julio) Betancourt, Robyn Hope (Donald) Pilarczyk and Adam (Janet) Tolan. Dear grandmother of Jason, Jared, Amanda, Jacob, Daniel and Shaina. Adored great-grandmother of Donavan, Joshua and Savannah. Cherished sister of Charlene (Yale) Simon, Fred (Pam) Forman and the late Maury Forman. Service Monday 12 Noon at Chicago Jewish Funerals, 195 N. Buffalo Grove Road (One block north of Lake Cook Road) Buffalo Grove. Interment Westlawn.
